Fill out the health questionnaire form and upload your medical records which are needed to verify your qualifying medical condition in order to obtain a MM certifcation.

Once the medical record review and in-office consultation is complete and eligibility is verified, you will receive your medical marijuana physician certification.

ATTENTION PROSPECTIVE PATIENTS

Patients with the following medical / psychiatric conditions may be eligible for medical marijuana certification. Cancer, positive status for HIV or AIDS, amyotrophic lateral sclerosis (ALS), Parkinson's disease, multiple sclerosis, damage to the nervous tissue of the spinal cord with objective neurological indication of intractable spasticity, epilepsy, inflammatory bowel disease, neuropathy, chronic pain as defined by 10 NYCRR §1004.2(a)(8)(xi), post-traumatic stress disorder or Huntington's disease or any condition for which an opioid could be prescribed (provided that the precise underlying condition is expressly stated on the patient’s certification). Patients must also have one of the following associated or complicating conditions: cachexia or wasting syndrome, severe or chronic pain, severe nausea, seizures, or severe or persistent muscle spasms, PTSD or opioid use disorder (only if enrolled in a treatment program certified pursuant to Article 32 of the Mental Hygiene Law).

If you have not been diagnosed with the above conditions, you are not eligible for medical marijuana certification. Please ensure that you have available all supporting medical records prior to your in-office visit consultation to avoid any delays in the certification process.
